Wildflower grows its support of patients and OB practices in Nevada with UNLV Health partnership

Wildflower and UNLV Health are partnering to provide patients with a suite of pregnancy and postpartum tools, which integrate into the provider’s current workflows.

The goal is to extend support to patients between appointments, while strategically delivering resources patients might need depending on clinical and SDoH risk factors. Providers also have access to actionable patient insights between visits and at the point of care.

The collaboration combines a mobile app with personalized health education and clinical support journeys, alongside Health Advocates for monitoring and escalations to the patient’s OB. These support tools from Wildflower are provided to UNLV Health’s OB clinics in Las Vegas with more than a dozen OBs and almost 3,000 patients in Southern Nevada.

Nevada preterm birth rates, infant mortality rates and gaps in health equity recently earned the state a D grade from the March of Dimes. Maternity care deserts are partially to blame for the state’s maternal health outcomes. More than half of Nevada’s 17 counties do not have a hospital or birth center that offers obstetric care.

Nevada Partnerships

These outcomes are a large reason why Wildflower is partnering with Nevada OB practices and health plans. Wildflower works closely with Women’s Health Associates of Southern Nevada (WHASN), a group of board-certified and board-eligible obstetricians and gynecologists with 20 care centers serving areas including Las Vegas, North Las Vegas and Henderson.

Wildflower also powers the Tummy2Family app for Health Plan of Nevada and UnitedHealthcare Health Plan of Nevada Medicaid. The alignment with these two UnitedHealthcare state-based health plans enhances the experience for WHASN and UNLV Health patients who are also health plan members.

Care Coordination

Patients are automatically connected to a digital app and Health Advocates via a shared platform that is integrated with the OB practice. Health Advocates deliver health education, care coordination, care escalation and remote monitoring, as needed. Health Advocates follow up with members to encourage adherence to physician care plans and provide referrals to health plan, OB and community resources.

Health Advocates also escalate members when urgent needs arise, such as a high maternal mental health survey score or high blood pressure. Depending on the need, escalations are routed to the health plan and the provider so the patient has the most comprehensive support.

All UNLV Health patients have access to a robust digital application that dynamically supports their evolving needs throughout pregnancy and postpartum. The application includes an extensive collection of evidence-based health articles and accompanying to do lists of important actions specific to mom’s health needs or current treatment. Health surveys and UNLV Health resources align to provide a personalized experience and easy connections to needed resources.

UNLV Health OB patients can get started by downloading the UNLV Cares by Wildflower app in the Apple App Store and Google Play Stores. Current patients will receive invitations from their OB to join the app to access additional services.
